At Angel House, we’re committed to providing high-quality recuperative care for individuals in need. As a trusted partner to agencies, we offer a safe and supportive environment for clients who require post-hospital care.

Referral & Connection:
A healthcare facility or hospital can refer patients who need ongoing care to Angel House.
B.
Assessment & Planning:
Our care team will evaluate the patient's needs and develop a personalized care plan.
C.
Welcome & Accommodation:
Patients will move into our facility and receive a private or semi-private room, along with a welcome kit.
D.
Comprehensive Care & Support:
While at Angel House, patients will receive medical supervision, medication management, meals, transportation, and other supportive services.
E.
Transition & Independence:
Our care staff will help patients connect with primary care, case management, mental health counseling, and other social services to help them achieve independence and secure housing.
F.
Follow-Up & Support:
Even after patients leave Angel House, we remain committed to their well-being and offer ongoing support and resources.
